Job Description:
As an RN in our Hospice Care team, you'll receive patient assignments from our Hospice Operations Leader.
Your focus will be on creating a positive work environment through effective verbal communication, and you'll take on the vital role of assessing, planning, implementing, and evaluating patient care.
Collaborative communication with attending physicians, hospice staff, and other agencies will be key to ensuring optimal care and resource use.
You'll keep a close connection with our Hospice Operations Leader for care reviews and maintain regular dialogue with attending physicians.
Your Responsibilities Will Include:
- Keeping meticulous patient records to ensure clarity and accuracy in the care process.
- Demonstrating your commitment to hospice philosophy of care, having at least one year of recent acute care/hospice experience.
Requirements:
- Currently licensed as an RN in the state of Illinois.
- A BSN is preferred.
- Proof of auto insurance for bodily injury and property damage.
